PURPOSE:

Provide direct assistance to children and adolescents with a behavioral health disorder, in gaining access to critical mental health, medical, social, educational, and other supportive resources within their community.


FUNCTIONS

  1. Provide Blended Case Management (BCM) services to a caseload that will not exceed 25 youth and their families, meeting minimum performance and productivity standards as identified by BCM Supervisor.
  2. Provide proactive outreach to youth and family in their home, school, and community and engage with, empower, and advocate for assigned youth and family.
  3. Conduct ongoing assessment of the youth and family's strengths and needs.
  4. Develop a comprehensive service coordination plan in collaboration with youth, family, and treatment team and routinely monitor progress.
  5. Provide linkage to identified resources with youth and family consent. This may include facilitating referrals for expanded mental health programming including EPSDT, IBHS, Psychiatry, FBS, etc.
  6. Serve as a clinical liaison and coordinate care between all service providers and systems serving youth and family. This may include but is not limited to: behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments.
  7. As appropriate, assist clients with applying for MA and Social Security applications or troubleshooting other insurance barriers that impact access to care. This may involve accompanying clients to appointments at Public Assistance and Social Security offices.
  8. Provide 24-hour crisis intervention on a rotating basis. This involves coordinating care with mobile crisis, the Philadelphia Children's Crisis Response Center (PCCRC), or psychiatric hospital. This may involve accompanying the youth and caregiver to the PCCRC or hospital.
  9. Ensure continuity of care when youth are moving from one level of care to the next to prevent gaps in service coverage. This is especially critical for youth moving from out of home placement (e.g., foster care, group home, RTF, AIP) back home.
  10. Actively participate in all staff meetings and provide consultation and support to team members as needed.
  11. Coordinate and lead interagency meetings for assigned cases for the purpose of service coordination and treatment and aftercare planning.
  12. Maintain required records and documentation in compliance with Agency, county, state, and Managed Health Care regulations.
  13. Maintains collaborative working relationships with service providers.
  14. Participate in professional development as assigned, including mandatory annual trainings and 10 hours of elective trainings per year as recommended by BCM supervisor.
  15. Function in compliance with agency policies/procedures and federal, state and local law.
  16. Participate in Continuous Quality Improvement (CQI) activities as requested by BCM Supervisor or Compliance Officer.
  17. Other duties as assigned.

QUALIFICATIONS:

  1. BCMs must meet one of the following criteria:
    1. A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
    2. Be a registered nurse.
    3. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in human services with 1 year in direct client contact, preferably with children.
    4. A high school diploma and 5 years of mental health direct care experience in human services with employment as an ICM staff person prior to 4/1/89.

2. Must have a valid Childline, PA Criminal History and FBI Clearances prior to commencing work.

3. Requires reliable means of transportation and valid driver's license from state of residence with a minimum of one (1) year driving experience.

4. Must be available to work some evening hours.
