Required Qualifications

- Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program.

- Prior experience in long-term care preferred but not required.

- Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the practicing state.

- Current CPR certification.


Major Duties and Responsibilities

- Direct the daily activities of certified nursing assistants in compliance with federal, state, and local regulations and facility policies.

- Ensure nursing personnel adhere to established policies and procedures.

- Participate in the admission, discharge, and transfer of residents as required.

- Observe changes in residents' status, notify the physician and family, and document accordingly.

- Transcribe physician orders to medical records and carry out orders as written.

- Prepare and administer medications per physicians' orders and observe for adverse effects.

- Perform wound treatments as per physicians' orders, observe changes, and document accordingly.

- Schedule follow-up appointments and transportation for residents as needed.

- Provide nursing leadership to assigned nursing personnel.

- Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, and IV administration.

- Perform rounds to ensure resident needs are met and personnel are performing assigned duties.

- Collaborate with the interdisciplinary team to holistically meet residents' needs.

- Inform appropriate departments of concerns or complaints and file grievances as necessary.

- Report incidents or unusual occurrences and participate in the investigative process.

- Participate in end-of-life care as required.

- Ensure adequate stock of medications, supplies, and equipment and notify appropriate personnel of needs.

- Initiate, review, and update care plans as required.

- Promote teamwork, mutual respect, and effective communication.

- Participate in the survey process during inspection surveys.

- Adhere to all facility policies, procedures, and state/federal regulations.

- Maintain confidentiality of protected health information.

- Report noncompliance with policies, procedures, regulations, or breaches in confidentiality.

- Report any allegations of abuse, neglect, or mistreatment of residents.

- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.

- Participate in life safety and emergency drills and training.

- Report work-related injuries and illnesses immediately.

- Follow established infection control policies and procedures.

- Complete all assigned training and skills competency.
