General Responsibilities
The primary purpose of the care manager position is to provide each assigned resident with routine daily nursing care and services in accordance with the residents' assessment and service plan and as may be directed by supervisor. Care managers are required to work cooperatively within the department as well as with other departments; report pertinent resident information to the immediate supervisor and respond positively to resident inquiries or requests. Position requires daily social interaction with memory care residents, ability, to conduct specialized programming as well as providing care.

a. Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
b. Confidentiality of all data, including resident, employee and operations data.
c. Quality Assurance and compliance with all regulatory requirements.
d. Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior.
4. Supports and participates in common teamwork:
a. Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.
b. Uses tactful, appropriate communications in sensitive and emotional situations.
c. Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns.
d. Promotes positive public relations with residents, family members and guests.
e. Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.

Requirements

Essential Duties
1. Provide quality nursing care to residents in an environment which promotes their rights, dignity, freedom of choice, and their individuality as illustrated by the following:
a. Provide individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental, and psycho-social well-being.
b. Knowledgeable of the individualized care plan for residents and provide support to the resident according to the care plan. Contribute to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents' needs and preferences.
c. Attend to the individual needs of residents which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, conduct specialized programming as designated on a daily calendar and other needs in keeping with the individuals' care requirements.
d. Maintain the comfort, privacy, and dignity of each resident in the delivery of services to them. Interact with residents in a manner that displays warmth and promotes a caring environment.
e. Fully understand all aspects of residents' rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
f. Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the charge nurse such as lighting or equipment problems.
g. Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items.
h. Observe residents for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel.
i. Provide care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin, etc.
j. Answer residents' call bells promptly and courteously.
k. Perform various tasks assigned by the charge nurse including vital signs, residents' weights, applying creams/ointments, collecting specimens, etc.
l. Assist with orienting residents and their families to the nursing home upon admission and to the unit when transfers occur.
m. Lift, move, and transport residents, using proper body mechanics or lifting devices for accident prevention.
n. Communicate and interact effectively and tactfully with the resident, visitors, families, peers, and supervisors.
o. Assist and escort residents for appointments such as beauty shop, activities, church services, etc. Participate in activities and functions as directed.
p. Practice careful, efficient, and no-wasteful use of supplies and linen and follow established charge

procedures for resident charge items.
q. Complete care manager records documenting care provided or other information in keeping with department policies.
r. Perform all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
s. Walk extensively to and from various locations of the unit while making rounds and transport residents within the facility.
t. Support an atmosphere, which promotes a homelike environment.
2. Respond to inquiries relating to his/her particular area or to requests from residents, visitors, other personnel, etc. within given time frames and within established policy.

Basic Qualifications
Minimum Education Requirement: A High School Diploma / GED Diploma.
Education/Training: Successful completion of a state approved HHA or CNA course or other education which meets state requirements.
Experience: A minimum of three months' prior nursing assistant experience in assisted living or long-term care preferred.
Working Conditions: Moves intermittently during working hours and subject to frequent interruptions. May be subject to hostile and emotionally upset residents, family members, personnel and visitors and must be able to cope with mental and emotional stress of the position.
Physical and Sensory Requirements: (with or without the aid of mechanical devises) Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel and support agencies. Position requires proficient interpersonal and communication skills, moderate reading, writing, grammar skills, visual and auditory skills; the ability to bend, stoop, sit, stand, lift, reach and push or pull a minimum of 50 pounds.
