As a Geriatric, Family Medicine, or Internal Medicine Physician, you'll have the opportunity to:
- Spend quality time with patients, digging into their health stories and truly understanding their needs inside and outside the exam room.
- Take a hands-on, team-based approach to care. Work closely with nurses, social workers, pharmacists, and other providers to develop comprehensive care plans tailored to each patient.
- Thrive in our unique value-based care model that prioritizes quality of life and improved outcomes for seniors. We go beyond the clinic walls to address social determinants of health.
- Enjoy strong physician leadership, mentoring, and a supportive work culture.
The role:
- Full-time outpatient only, no weekends or on-call
- Patient panel of 12-15 per day
- Emphasis on preventive care, chronic disease management, care coordination
- Opportunity for leadership and development
- Must be Board Certified or Board Eligible
