POSITION SUMMARY:

Reporting to the CDPP Program Manager and working collaboratively with the HIV RN Case Manager, the CDPP Navigator performs a variety of direct and indirect patient care and population management activities. This position provides specialized support in performing the functions as a medical assistant. Specialized support includes but is not limited to completing HIV intake forms, assisting with clinical sessions including rooming patients and assisting with procedures and clinical services as appropriate and at the discretion of the HIV RN and MD. The CDPP Navigator will help with maintaining individual HIV clinical flow sheets, case review forms for case management meetings and in collaboration with the HIV RN and Data Analyst helping with HIV program population health management. The CDPP Navigator promotes patients self-management and empowers patients/families to achieve maximum levels of wellness and independence. The Navigator will actively engage in care team huddles, clinical meetings, and quality improvement activities.

The CDPP navigator will work with the X Clinic team to assist during clinical sessions including at times rooming patients, cleaning rooms and assisting with data entry, supply management and room stocking.

PRIMARY RESPONSIBILITIES:

  • Routine Patient Care
    • Rooming patients
    • Collect data, information and vital signs pertinent to visit (examples include blood pressure, pulse oximetry, chief complaint or a sexual health risk assessment)
    • Perform screenings pertinent to visit.
    • Assist with interaction between providers, co-workers and patients as needed.
  • Preparation
    • Prepare exam room for correct procedure.
  • Clinical Support
    • Assist providers as necessary.
    • Obtain specimens as directed.
    • Perform Point of Care Testing according to protocol
  • Worksite
    • Monitor and maintain stock supplies and equipment in clinical area
    • Maintain a safe patient care environment
  • Population Management
    • In collaboration with HIV RN and CDPP Data Analyst maintaining and monitoring registries of patients with HIV and STIs to ensure scheduling of appropriate follow-up and to monitor and meet quality metrics and grant requirements.
    • Assist with scheduling referrals and transportation needs for patients.
    • Utilize database to identify patients overdue for care (e.g. lab monitoring or follow up visits) as established by licensed clinical staff and collaborate with HIV RN and X Clinic Navigators to engage patients in completing items needed.
    • Participation in case management meetings and trainings for HIV teams.
  • Clinical Assistance
    • Initial clinical screening of patients requesting or referred for HIV services or HIV preventive services.
    • Provides MA support during HIV sessions when needed including rooming patients, vitals, collection of specimens, obtaining outside records.
    • In collaboration with HIV RN, helps maintain the HIV clinical flow sheet document in each chart.
    • Assists HIV RN with prior authorization and other patient related forms.
    • Collaborate with HIV Medical Case Managers to assist with schedules, forms and patient outreach.
  • Other activities within the scope of the position as designated by CDPP Program Manager or designee.

Qualifications

  • Experience as a Medical Assistant (minimum 2 years); graduate of a certified program.
  • High School diploma or equivalent.
  • Computer experience a plus,
  • Outstanding customer service skills a must.
  • Experience working with and sensitivity to ethnic, cultural, gender and stigma issues related to at risk populations for HIV.
  • Knowledge and prior experience with LGBTQIA+ populations
  • Familiarity with Electronic Medical Records (EMRs) and Electronic Health Records (EHR) systems
