Job Summary:
In accordance with the organization's mission, vision and values, the Alerts Coordinator is responsible for the tracking and dissemination of information entered into and obtained through data platforms such as SPARS, RHIO, PSYCKES, e-Intervention, UniteUs, etc for information that supports quality client care and coordination of services. Care Coordinator will additionally assure agency and program compliance and fidelity in strategic business goals and will be expected to provide reporting to multiple departments.
Minimum Qualifications:
Associates Degree in Health/Human Services with a t least one year of working experience preferred. Pursuit of QHP is encouraged. Valid NYS Drivers' License. Experience and/or training in Behavioral Health/Client Care.
