POSITION TITLE: Reunification House Case Manager

DEPARTMENT/PROGRAM: Counseling Department

REPORTS TO: Clinical Supervisor or Clinical Director


SCHEDULE: 8:00 a.m. to 4:30 p.m. Monday, Tuesday and Friday

10:00 a.m. to 6:30 p.m. Wednesday and Thursday

Flexibility required for emergent needs



GENERAL STATEMENT OF DUTIES: Provides case management and support to families living at the Transitional Reunification House.


ESSENTIAL RESPONSIBILITIES:

1. Provides case management and support to families living at the Transitional Reunification House which includes creating a case plan for each family residing in the home.

2. Coordinates and teaches family enrichment and adult services workshops to the residence of the house which includes bi weekly AA meetings and the following classes: parenting, family enrichment, counseling, like skills/education and training, budgeting/goal planning and health and wellness.

3. May provide child care for youth residing in home for sessions that the case manager does not teach or facilitate.

4. May provide transportation to and from school and appointments for adults and children residing in the home.

5. Ensures residents sign in and out for workshops and are compliant with the policies and procedures associated with being a resident in the transitional home.

6. Conducts, at minimum, weekly case management sessions with the residents in the home to go over goal plan/case plan objectives.

7. Maintains regular communication with Portage JFS on progress which includes submission of a bi monthly progress report.

8. Attends agency meetings including Portage JFS meetings, community meetings, and court hearings as requested.

9. Notifies Portage JFS immediately when emergencies arise with families residing in home.

10. Complies with Mandated Reporter requirements.

11. Provides community education and awareness to the community and community partner organizations.

12. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.

QUALIFICATIONS:

1. Associate Degree or two years related field experience preferred

2. Ability to meet agency requirements for criminal background check.

3. Must possess a valid Ohio driver's license in accordance with County policy and must maintain motor vehicle liability insurance, must maintain continuing eligibility under the existing County driver eligibility standards.

4. Ability to function as part of an organization by working with others and by adhering to established policies and procedures.

5. Training in First Aid, CPR and De-escalation techniques.


MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Bachelor's required or four years of related experience in social service discipline.


PHYSICAL REQUIREMENTS: Must be able to travel to and work in clients homes.
