Case Manager

5 month assignment. Mon-Fri with rotating weekends and holidays. Computer skills. Epic experience, Discharge planning experience, Utilization Planning experience and PRI Screener required. NYS RN License required. Case Management Certification preferred. All Scrips experience (P)At least 5 years clinical experience as a registered nurse in a medical or surgical unit, Home Care or post-acute facility experience in critical care preferred.
