Without them, I'd be waiting somewhere, waiting for someone to give me a chance to live " - Client from Telecare What You Will Do to Change Lives Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Shifts Available: Full Time ; 8:00am - 4:30pm ; Monday - Friday Expected starting wage range $17.30 - $22.00 Hourly. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. What You Bring to the Table (Must Have) High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services Valid and current driver's license, and personal vehicle insurance with your name listed as a driver. Program Type: Community-based To Make Referrals: Telecare Gateway to Recovery ACT is a voluntary closed-referral program that assist adults and Transition Age Youth (TAY) in their recovery as they transition from long term locked treatment facilities. They also serve persons who are identified on San Diego County's current High Utilizer List. of Members: 235 members Who We Serve: Must be a resident of San Diego County Adults and Transition Aged Youth(TAY) with Medi-Cal Must have a qualifying Title IX diagnosis and may also have co-occurring substance use diagnosis A county referral for persons stepping down from a long-term care facility or a person on San Diego County's high utilizer list Long-term care facility diversionary referrals may be considered at the County Monitor's discretion About the Program: At Gateway to Recovery, we believe in a whatever-it-takes approach to empower your recovery from within.
