Case Manager Clinician - Children's Residential Services (Requires LLMSW, LMSW, LLP, TLLP, LPC, or LLC)

What you'll be doing

Clinical Responsibilities

  • Provide comprehensive psychosocial assessments.
  • Perform individual therapy that is suited to the needs of each child. A minimum of one hour of individual therapy is required.
  • Meet with all kids on case management case load at least weekly. Average case load: 10
  • Coordinate and ensure child involvement in the Person-Centered Planning Process (PCP) for assigned consumers.
  • Assure responsibility for the development & implementation of each assigned child's Individual Plan of Service.
  • Link children to necessary ancillary services.
  • Act as an advocate and/or a liaison coordinating services among providers of service and referral source.
  • Maintain children's records - meeting all appropriate standards according to HNBHS and all regulatory bodies.
  • Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  • Facilitate the coordination of children's intakes, discharges, and transfers to and from other facilities programs, agencies.
  • Facilitate communication with guardians, payees, and family members regarding children's care, financial and treatment issues.
  • Provides consultation to direct care staff regarding clinical and program issue, where applicable.
  • Participate in children's program reviews and team meetings, where applicable.

Administrative Responsibilities

  • Participate in committees, task forces, and other related meetings and activities as assigned by supervisor.
  • Practice according to licensing and accreditation for assigned program(s).
  • Attend agency meetings: clinical, administration and management.
  • Network with relevant agencies and providers as needed.
  • Follow general safety practices. Report any unsafe conditions to the manager of that location.
  • Complete and submit Service Activity Logs (SAL's) in accordance with agency policy/protocols.
  • Coordinate benefits and verify benefit status monthly.

Anticipated Work Schedule

  • Monday - Friday 8:00am-5:00pm (flexible schedule)

On Call Rotation (frequency varies on location)

  • One week per month

On Call Compensation rates:

  • Additional $50 for on-call duty during Monday to Friday
  • Additional $100 for on-call duty during Saturdays and Sundays
  • Additional $150 for on-call duty on holidays

Job Requirements

  • Master's in Social Work, Psychology, or Counseling Degree.
  • You possess or are in progress to receive the one following licenses: Licensed Master Social Work (LMSW) OR Limited Licensed Master Social Work (LLMSW), Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), OR Temporary Limited Licensed Psychologist (TLLP), Limited License Counselor (LLC), or Licensed Professional Counselor (LPC).
  • We will consider candidates who are currently in the process of obtaining licensure or are about to commence the licensure process with LARA.
  • Must complete a Central Registry Clearance form for the State of Michigan and any state in which residence and/or employment was previously established. Michigan Central Registry Clearance must be completed on an annual basis.
