Client location and schedule hours:
Female client in Nashville. The schedule is Monday 9 am to 2 pm. Wednesday 9 am to 2 pm. Friday 9 am to 5 pm.

Job Responsibilities:
Social History: Client is an 84yo widow, she lives with her only daughter. Her memory and functioning have noticeable declined in the past 2 weeks. Client enjoys watching Channel 5 news & QVC channel. She likes looking at magazines and going on walks or sitting on the back deck if the weather is nice. Client needs a caregiver who will engage with her and assist her with toileting and showering and laundry/bed linens. Client is sweet natured and converses well. The family has 3 little dogs.


Diagnoses: Dementia (mid-late stage), Congestive Heart Failure, Arrhythmia, Pacemaker, Osteoporosis (brittle bones), Cracked Pelvis, Fall risk due to Weak knees.


Goals & Objectives: Engage with client, offer to assist her in the toilet as needed, undressing, wiping & redressing with Depends, assist with showering, undressing & washing her hair. Remind her to use her walker when needed. Offer drinks to stay hydrated. Heat up snacks or meals as needed. Light housekeeping. Check bed every morning and change sheets/pads as needed. Walk outside with her or sit outside on deck when weather permits and discuss nature. Watch TV with her or look at magazines with her. Med reminders if not taken yet at shift start and at bedtime if there

Mobility: Client should use a walker at all times unless she is going longer distances, then she needs a transport chair or wheelchair. She is slow to get up and down but can do it by herself. She needs a little help getting into a vehicle. Observe for weakness to rest appropriately

Bathing/Dressing: Needs encouragement to get in shower, so attempt at every shift. Turn on water til appropriate temp for her. Assist with undressing and stepping into shower. Hand her wash cloth & let her wash herself a bit offering to wash her back. Hand her the shampoo & she will wash hair but needs help rinsing. Assist out of shower and to dry off. She needs help choosing clothing

Toileting: Client needs to be encouraged/asked to go to the bathroom several times on the shift. She may decline but keep asking. She may need help pulling down her pants and wiping but ask if you can assist first. Needs to be reminded to put on fresh depends as needed. Needs help flushing and reminded to wash her hands after.

Eating: Client can feed herself. Client is a light eater. She may eat soup or a sandwich for lunch and needs reminder that it is time to eat. She can drink by herself and use utensils. Observe if she begins to have difficulties putting food onto fork or into her mouth.

Communication: Client's speech is clear and articulate. She can engage & exchange minimal conversation. She is most often confused either about the day or time or what she likes to eat.

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
