Your Role as a Care Manager RN:

As a Care Manager RN, you will play a pivotal role in:

  • Collaborative Care: Partnering with patients, families, nurses, social workers, physicians, and other stakeholders to create a seamless and supportive care experience.
  • Communication and Coordination: Facilitating communication and care coordination across the continuum, ensuring effective transitional care management.
  • Patient-Centered Advocacy: Recognizing the patient's right to self-determination and addressing the social determinants of health, you will champion patient needs and preferences.
  • Holistic Care: Understanding the complexities of care, you will address barriers to coping, abuse, mental health concerns, substance use, lack of resources, and psychosocial issues.

Qualifications:

  • Required:
    • Bachelor's Degree or equivalent education/experience.
    • California Registered Nurse License upon hire.
    • 2+ years of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute).
    • 2+ years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Managers. TIP candidates must have experience in the same type of nursing unit where the CM position is available.
  • Preferred:
    • Bachelor's degree in nursing.
    • Master's degree in a healthcare-related field.
    • Certification in Case Management (ACM or CCM) upon hire.
