JOB SUMMARY
The role of the Memory Care Program Coordinator is to provide opportunities for our residents to enjoy the highest possible quality of life through meaningful engagement in a real home environment that recognizes and honors the residents and their wishes and interests. The Memory Care Program Coordinator designs, implements and executes programming that honors rich and meaningful lives for our residents. The Memory Care Program Coordinator is a servant leader who creates relationships with residents, their family members and staff in a manner that is respectful and collaborative. The Memory Care Program Coordinator reports to the Wellness Director. This is a safety sensitive position.

ESSENTIAL FUNCTIONS

  • Design, create, provide and monitor activities of therapeutic value that encourage active participation from all residents
  • Maintain accurate daily participation records on each resident and ensures goals are in their resident care plans
  • Evaluate efficacy and effectiveness of treatment interventions related to client progress
  • Monitor programs effectiveness through quality assurance procedures
  • Maintain the confidentially of pertinent records
  • Set priorities and allocate staff and other resources to meet needs in a timely, effective manner, and establish and maintain staff and work schedules to meet coverage needs
  • Create and maintains procedures
  • Review statutes, legislation, rules, regulations and policies and recommends procedural changes as appropriate
  • Collaborate and coach care staff to develop solutions to resident behaviors
  • Guide care staff on therapeutic activities they can provide to residents
  • Plan and manage budget
  • Complete prep work required to perform scheduled activities
  • Provide off-site activities within the community and arranges for transportation
  • Participate in in-service and educational trainings and programs
  • Regular and reliable attendance is an essential function of this job

ESSENTIAL MENTAL AND PHYSICAL FUNCTIONS

The essential mental and physical functions listed below are required in order for an employee to be able to perform the essential functions of this position.
  • Ability to read and write, follow written and verbal instructions, and communicate effectively in English.
  • Thinking critically; defined as reading, understanding, and taking appropriate action on detailed and complex information.
  • Walking long distances regularly.
  • Lifting and carrying up to 50 lbs.
  • Pushing a wheelchair with a resident weighing up to 200 lbs.
  • Repetitive actions/motion of one or both hands.
  • Hearing ordinary conversation and office sounds.
  • Verbal communication in person and on the phone.
  • Written communication by hand and on PC.

WORKING CONDITIONS

Work is conducted primarily in a senior living community and its surrounding grounds. This position involves a wide variety of environmental and atmospheric conditions. This position frequently requires working within strict deadlines, under stressful conditions and with a variety of personalities, who may at times be emotional.

Qualifications

MINIMUM QUALIFICATIONS
  • Bachelors degree from an accredited four-year college or university
  • Nationally Certified Therapeutic Recreation Specialist a plus
  • One or more years of related experience and/or training in geriatrics in long term care, home health or other health care setting
  • Excellent verbal and written communication skills
  • Computer literacy which includes, but is not limited to, Microsoft Office suite
  • Outstanding communication and interpersonal skills
