Salem Community Schools Position: BSE Corporation Nurse Minimum Qualifications: Must be licensed as a registered nurse in the State of Indiana and good standing with the Indiana Board of Nursing Demonstrate adequate verbal and written language skills and serve as a role model for adults and students. Must have good moral character, ethical code, and sufficient physical and mental abilities to perform duties. Ability to communicate effectively with adults and students orally and in writing Must be able to sit, stand, bend, squat, and lift to 30 lbs. BSE Duties may include but are not limited to: Manage and coordinate the assigned school's health services program based on requirements established by school division policies, procedures, protocols, and local, state, and national regulations and statutes. Serves as the health advocate for all school children, focusing on promoting healthy living styles and preventing illness and disability. Excellent organizational skills Ability to communicate with children, their parents or guardians, and teachers. Administer medications Maintain parental consent forms for medications. Record medications as given. Locate any student who does not come to the Health Center when medicine is scheduled to be given. Evaluate and treat students with injuries and/or illnesses. Contact parent when indicated. Supervise students in the clinic until a parent arrives. File accident forms when indicated. Administer as-needed medication when appropriate (i.e., asthma medications and Epi-pens for allergic reactions.) Immunizations-Kindergarten Early in the year, check each student's record. Record immunizations in permanent school files, computer files (Power School) Send notice to parents regarding insufficient immunizations. Send follow-up notices as needed. Send reminders to parents anytime during the school year when a child is due for a booster. File a report with the State of Indiana regarding immunization status. Send out exemption forms to all students who don't receive immunizations. Send information home to parents concerning meningitis. New enrollees and transfer students Check records for grade-appropriate immunizations. Notify parents of any incomplete immunizations. File report at year-end with the State of Indiana regarding immunization status Pediculosis (Head Lice) Check any student suspected of having head lice when sent by a teacher to be checked. If lice or nits are found, contact the parent to take the child home for treatment. If live bugs are found, check siblings in school so they may go home with their parents. Check students in the classroom who are found with lice. A nurse must check any student sent home with head lice to verify proper treatment before returning to class. Vision Screenings Schedule First-grade vision screening with Optometrists. Prepare forms for each student in first grade and students in MoMh class. Provide vision screening to all Third and Fifth-grade students. Provide vision screening to any student referred by teachers or parents. Send referral letters home to all students found to need further vision care follow up in 30 days. Record results of vision screening on student permanent records and computer files (Power School) File the year-end report regarding vision screenings with the State of Indiana. Assist LPNs at other schools with vision screening if necessary. Kindergarten Registration Provide parents with a list of immunizations needed before starting school. Provide parents with the necessary forms for physical and dental exams. Supplies Order health center supplies for all schools and buildings. Maintain supplies at the elementary building and dispense to other buildings as needed. Buildings other than Elementary Respond to high school, middle school, and alternative school as requested for emergency care, for serious illness, etc. Major Health Concerns-Students Compile and maintain files on students with chronic illnesses. Must have action plans in place. Prepare Individualized Health Care Plans and Services for students with disabilities and health conditions that interfere with learning.
