Screening and Planning: 40%
•  Implements risk assessments and healthcare pathways for the breast program based on the developmental stages of the patient. Involves family/significant others in plan of care and conducts patient and family education as appropriate, considering the developmental needs of the patient. Acts as a resource to interdisciplinary staff in the care of Breast Program patients Delivery of patient care including assessment, planning, implementation, and evaluation of patient outcomes Provides support, education, referrals, coordination of care and other assistance during the diagnosis and treatment planning stages Serves as point of contact for Oncology and Surgery B.Care Coordination: 40%
•  Facilitates the entry of patients into the Breast Program and coordinates their care. Follows the patient throughout the continuum of breast care services. Monitors healthcare pathways with care teams and intervenes when necessary Coordinates patient care, and acts as a liaison between physicians, i.e. radiologists, primary care physicians, general surgeons, pathologists, oncologists, plastic surgeons, and radiation oncologists who provide care for the Breast Program patients. S/he provides information and support to those patients facing the reality of a breast cancer diagnosis and path forward related to treatment options, social-emotional supports, access to integrative therapies, and other resources, providing continuity throughout treatment and into survivorship Collaborates with medical, nursing and mammography staff in data collection and statistical activities to facilitate problem identification and resolution to improve quality of patient care C. Leadership: 20%
•   Assists with implementation of policies and programs Serves as a resource for staff Effectively communicates with medical, clinical, and administrative staff to ensure efficient operation of the department Effectively communicates with physicians and visitors May be required to occasionally attend work events on evenings or weekends Assumes responsibility for own professional growth by attending continuing education classes, seminars and reading professional journals D. Exhibits computer systems knowledge and proficiency as necessary to perform job functions. E. Demonstrates the attitudes and behaviors of the South Bend Clinic's Service Standards. F. Adheres to HIPAA guidelines set forth in Clinic policies and procedures. G. Performs other duties as assigned when appropriate. Qualifications The Experiences You Bring Education/Certification/License: Associate Degree or higher is required. Current RN license to practice professional nursing in Indiana. Oncology experience with certification preferred Breast Care certification preferred BLS preferred Knowledge, Skills, and Abilities: Five (5) years nursing experience required Ability to provide order and structure to daily processes and work environment and demonstrates good organizational skills, and ability to prioritize daily work Communication skills which excel in ability to interact with and relate to, people of varying educational levels and backgrounds, conveying information clearly and succinctly, applying listening, tact, responsiveness, empathy, and confidentiality. Effective in communicating verbally with other staff and departments related to all aspects for patients across the continuum of care Effective skillset in identifying and analyzing problems Ability to pay attention to detail and thorough in completing tasks Proven teamwork strength displaying ability be able to get along with others, work as part of a team, accept constructive criticism, adapt behaviors quickly, and consistently follow and apply work rules. Works effectively with others to accomplish objectives and goals. Willingly aid others when the need arises. Fosters teamwork and positive rapport within all departments to maximize achievement of goals Strong computer proficiency and ability to learn and become proficient in Nextgen as well as have basic knowledge of Microsoft office applications, i.e. Outlook, Word, Excel, etc. Special Demands/Requirements: None
