Position Summary Provide effective culturally competent behavioral health assessments and therapy to individuals, families and groups in a community-based in-person Primary Care setting. Works flexibly as part of an integrated primary care team and promotes integrated behavioral health.
Key Functions and Responsibilities - Maintain confidentiality of records relating to patients' treatment.
- Provide mental health and substance abuse assessments.
- Provide Individual, Couples and family short-term mental health treatment for adults, children and teens using evidence-based interventions such as CBT, MI, trauma informed care and/or Solution-Focused Brief Therapy
- Relates to a wide variety of people including members of ethnic and sexual minority populations and is respectful of individual and cultural differences.
- Conduct substance abuse screenings and brief counseling or referral for addictions treatment.
- Develop and implement treatment plans based on patients' expressed goals. Complete and maintain client- related records in the EMR (ECW), daily.
- Follow regulations for documentation and billing.
- Collaborate and/or consult with other team members to perform clinical assessments or develop treatment plans.
- Coordinate clinical approaches with the medical team.
- Refer patients or family members to community resources or to specialists as necessary.
- Manage patient crises as needed. Modify treatment activities and documentation to comply with changes in patient's status for accreditation or regulatory requirements.
- Counsel family members to assist them in understanding, or supporting patients, as appropriate.
- Monitor patients' use of medications and encourage their collaboration with prescribers.
- Learn about new developments in counseling and health by reading professional literature, attending courses and seminars, or establishing and maintaining contact with other social service agencies.
- Coordinate or direct employee workshops, courses, or training about mental health and substance abuse screening and treatment.
- Provide leadership in integrated care team collaboration.
- Successfully meet productivity targets including group therapy or clinical supervision requirements
- Develop cultural competence to work with a diverse population including refugees and asylees.
- Provides office-based appointments.
Minimum Qualifications - Must be able to provide proof of independent licensure
- Licensed Clinical Social Worker (LCSW-C),
- Licensed Psychologist
- Licensed Clinical Professional Counselor (LCPC)
- Minimum 2 years providing therapy to children, teens and adults with mental health needs.
- Excellent team-based communication skills with primary care team, supervisors and other CCI staff.
- Ability to define problems, collect and interpret data, establish facts, draw valid conclusions, and process work to completion.
- Preferred Bi-lingual (Spanish, Amharic, Pashto) capabilities in reading, writing, and speaking preferred and understanding how to use interpreter services.
- Ability to develop therapeutic rapport and engage patients in treatment using clinical skills including Motivational Interviewing in a culturally based manner.
- Ability to provide crisis intervention services to patients, an ability to be flexible, use new ideas, and operate in a fast-paced health environment.
- Prior experience collaborating with physicians and other health care providers, social service agencies, and/or education system desired.
- Enthusiasm for innovation, data driven decisions, clinical and financial outcomes.
- Other duties as assigned.
