JOB SUMMARY

Under the supervision of the Behavioral Health Supervisor, responsibilities include individual and group level mental health screenings, assessments, counseling and referral(s) of all clients receiving services within Project VIDA's Behavioral Health program. Direct service team consultations and education regarding mental health issues of both dual program clients and mental health issues of the community at-large. Facilitation of support group focusing on various topics such as depression, anxiety, anger management, stress management, medication adherence and development of social supports.

JOB DUTIES

Secures clients' medical, psychosocial, and wellness/lifestyle information and evaluates these

areas through comprehensive biopsychosocial assessments.

Conducts intake sessions as back-up to Intake Specialist and other program staff and maintains paperwork in confidential client charts.

Develops individualized treatment plans for each individual client and maintains updated notes and plans in client database and charts

Conducts support group for Spanish-speaking clients, assists in planning speakers and topics for group

Provides referrals and documents follow-up for referrals

Conducts outreach sessions as needed and in conjunction with program staff to recruit new clients

Assumes responsibility for the accuracy, substance and timeliness of program reports, projects, schedules and other relevant work products.

Submits reports to Manager of Direct Services and Chicago Department of Public Health within the designated time lines

Enters data in client-level database system and submits reports to Manager of Direct Services and funders according to established deadlines

Maintains an appropriate professional demeanor toward co-workers, visitors, clients, supervisors, and all others

Adheres to professional standards as outlined by governmental bodies, funding sources, organization plans/policies and program guidelines

Participates in periodic evaluative reviews and in-house and all required external staff training and meetings

Can serve as an advocate for clients' needs, keeping clients informed of new policies, services, and other events that can help enrich their well-being

Perform other duties as assigned

The work schedule would be alternating Monday through Friday from 9 am to 6 pm one week and Tuesday through Friday from 11 am to 8 pm and Saturday from 9 am to 6 pm the next week.

QUALIFICATIONS

Master's degree in counseling, psychology, social work or related field preferred

Clinical Licensure (LPC, LSW) preferred.

Knowledge and previous work in the HIV care field strongly preferred.

Fluent in English and Spanish required.

Cultural/lifestyle sensitivity to African American, Latino, and LGBT populations required.

Strong interpersonal skills and ability to work within a diverse team.

Presentation/group facilitation skills.
