The Behavioral Health Home (BHH) team at Woodfords Family Services is searching for qualified BHH Coordinators (Children's Case Managers) to join our growing team of dedicated professionals. These are full-time positions (32-40 hrs. per week) and based at our office in Manchester, ME. Position Summary The BHH Coordinator is an integral part of the team and is responsible for working with children with disabilities and their families to provide support, advocacy, and connection to services that encourage and promote family empowerment and self-sufficiency. This will be achieved through a variety of channels including: Assessment and evaluation, plan of care development, maintenance and review, and facilitating and attending family/team meetings. Responsibilities: Meets regularly with child and family in a variety of settings to assess consumer functioning, progress, quality of life and health and safety issues, and to address any concerns identified. Possess knowledge regarding community resources and assists families with referrals to such resources. Conducts strength and needs assessments for all children and facilities family wrap around meetings as needed. Serves as a liaison with other agencies involved in providing other types of services to the child (public schools, behavioral health agencies, consultants, etc.) to ensure appropriate level of care and where deficient, direct additional resources. BHH Coordinators receive lower productivity requirements AND additional incentives starting at $350. Schedule & Compensation Full-time, 32-40 hours per week Monday through Friday - Flexible hours & schedule. $42,500-$46,000 annual salary (commensurate with experience) $1,500 Sign-on Bonus. Paid at 6 months of employment.
