POSITION SUMMARY The Behavioral Health RN Coordinator will lead the coordination of services within Riverwood Behavioral Health Clinic. Responsible for providing program and staff oversight and professional nursing care for patients, including assessment, treatment, care planning, and evaluation by following established standards and practices. This role works collaboratively with others internally and externally to restore health and promote wellness through coordinating comprehensive nursing care for patients of all ages, gender, economic and social status, and backgrounds. POSITION QUALIFICATIONS Education and Experience Graduate of an accredited School of Nursing with an associate degree or higher. 1-2 years of experience providing direct care to patients of all ages. 1-2 years of clinical experience in mental health and/or similar health care setting preferred. Experience with Epic EHR strongly preferred. License/Certificates Current, unrestricted RN Licensure within the State of Minnesota required. BLS required. ESSENTIAL DUTIES RESPONSIBILITIES Provide direct support to our medication management providers, therapists, clinical assistants, and behavioral health assistants. Work collaboratively with physicians, advanced practice providers and other multidisciplinary team members internal and external to Riverwood Healthcare Center. Serve as the bridge between primary care and behavioral health clinics. Coordinate activities, handouts, and other initiatives with PR and Marketing to ensure all marketing materials and promotion adhere to established brand standards. Facilitate team meetings to support day-to-day clinic flow and operations. Manage referrals detailing relevant mental health histories for provider review. Play a critical role in communicating with staff and interdisciplinary care teams; first line of contact for staff regarding concerns or need for problem-solving. This includes staffing and scheduling conflicts. Provide care coordination for through clinical assessments, developing individual care plans, health coaching, and evaluating the effectiveness of interdisciplinary care team support. Serve as patient advocate and support coordination of care needs including transportation, appointments, and connecting patients to appropriate community resources. Assist with patient transfers to hospital or emergency department. Accurately perform duties and responsibilities related to prescription refills, sample management and medical supply requests using established nursing practices and adopted protocols in an efficient and timely fashion. Works as a liaison between Pharmacies and Providers, answering questions and clarifying orders. Coordinate and assess patient inquiries. Provide timely telephone care coordination for patients with health concerns or questions. This may include coordination with pharmacies or other healthcare establishments. Utilize nursing process and approved decision-making protocols to assess the health care need and urgency of patient inquiries, based on reported symptoms and history. Independently address concerns/questions within RN scope of practice; involving the Provider(s) either emergently or as needed for issues requiring their involvement Provide or facilitate transfer of care to the appropriate level as applicable, including but not limited to health education, scheduling a clinic visit; or referral to a same-day clinic, emergency department, or 911. Document all contacts and inquiries thoroughly and accurately in the patient electronic health record (EHR) for efficient future referencing and trending. Prepare for, attend, and participate in meetings as assigned. This includes internal meetings and relevant community partner meetings. Responsible for training of other personnel in the performance of nursing activities, which may include competencies, chart reviews, and performance review feedback. Assist with quality improvement and population health outreach activities as assigned. Assist with policy, protocol, and standing order development as assigned.
