Job Description:

The Clinician will perform a variety of duties as a member of the Patient Centered Medical Home, Primary Care Team. Work will include consultations, brief
assessments, brief interventions, care coordination, and individual, family, and group behavioral health interventions with children, adolescents, and
adults. The Clinician will work directly with medical providers and the care team to collaboratively address patients' care and to support optimal medical
and behavioral health. The counselor will engage in and support patient access, high quality care, and overall productivity
of the Team.

Educational Requirements:

Master's degree in social work, mental health counseling, psychology, or a closely related field, required.


Experience Required:

Two to four years direct patient care and managed care experience preferred; public sector experience may be
applicable. This experience may be in an internship. Demonstrated knowledge of
collaboration needed to achieve common objectives of medical/behavioral
healthcare preferred. Public sector
experience may be applicable.


Professional Licensure/Special Skills and Certification Required:

Candidate must be eligible for and actively working towards obtaining an LICSW or LMHC, or other similar credentials.

3. Ability to deal with sensitive information in a highly confidential manner.

4. Effective communication skills, both oral and written, with patients, families, visitors,
co-workers, health care professionals, funders and other regulatory agencies.

5. Bilingual in English and another language appropriate to the health center's patient
population preferred, but not required.

6. Ability to function independently with minimal supervision, and to plan, coordinate,
and carry out those tasks necessary to meet the job responsibilities.

7. Awareness of resources available within the health center to accommodate the needs of
patients.

8. Commitment to providing cost conscious, fiscally responsible, timely, efficient,
effective, safe, patient-centered, culturally competent, equitable, and
participatory health care.

9. Processes personal health information in accordance with FHCW HIPPA policies and
procedures.


Reporting Relationship:

Reports directly to the Manager of Integrated Behavioral Health
