The Behavioral Health Clinical Care Coordinator is responsible for the provision and management of patient care in a clinical area for a behavioral health population. To manage the plan of care for a caseload of patients. To serve as the liaison with the multidisciplinary care team. To plan care management as achieved through coordination of care with physicians, nursing staff, counselors, and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources. Coordinates patient follow-up clinic visits, verifies appropriate criteria. Under the general direction of a licensed mental health/substance abuse provider coordinates care of patients, serving as a resource for patients, families, physicians, and other community health partners in the delivery of care. Obtains information regarding referral source to assist in continuum of care or return to referral source for continued care. Provides patient / family education regarding care, medications, and access to additional resources in the continuum of care. Utilizes critical thinking abilities in evaluation of care and the triage of patients. Displays professional rapport with all patients, family and faculty and staff. Displays competencies for basic computer skills, which include MS Word, Outlook, and electronic EHR. Performs other duties in support of specific departmental functions and as deemed necessary. Position Requirements: EDUCATION AND EXPERIENCE: Bachelor's Degree and two (2) years' experience working in the Substance Abuse community TRAITS & SKILLS: Must be self-directed / self-motivated; must have good communication and interpersonal skills.
