We are hiring IMMEDIATELY for a BEHAVIORAL HEALTH CASE MANAGER to join one of our community mental health providers.

This position offers a sign-on bonus of $10,000 & $3,500 Relocation.

Salary: $60K - $92K.

Job Summary:
A rural behavioral health agency on the Oregon Coast seeks a full-time Behavioral Health Case Manager. This position does offer a sign-on bonus. The clinician will work within an agency team, coordinating client care and providing direct clinical services to individuals with significant behavioral health concerns. Under the supervision of the Assertive Community Treatment Team Lead/Supervisor, this provider will provide treatment services and support to individuals in navigating complex health and community agency systems toward improved health outcomes.

Duties and Responsibilities:
Partners with agency clinicians and providers to assist high-needs clients to navigate complex systems such as health services, housing, legal needs; assists in connecting with resources that can improve life quality and health outcomes.
Coordinates plans of care, in collaboration with local and statewide community partners and stakeholders (Senior and Disability Services, Behavioral Health Network providers, complex care coordinators).
Functions as the Lead Substance Use Services provider for the agencys Assertive Community Treatment Team (ACT).
Partners with licensed adult foster homes on behalf of individuals who are served by the agency.
Completes Behavioral Health Assessments for program referrals; diagnosis behavioral health conditions (mental health and substance use); develops and coordinates treatment plans as appropriate.
Completes all required agency documentation within the designated electronic medical record system (CareLogic) in a professional and timely manner.
Serves on the agency crisis team as part of an on-call crisis rotation providing crisis screening and community crisis intervention.
Meet routinely with the Clinical Supervisor to review client caseload and job expectations.
Attend all assigned training and agency meetings, as required.

Schedule:
This is a full-time position, 40 hours per week. Days and hours of work are variable, but generally Monday-Friday, 8am-5pm. This job may require longer hours and a flexible schedule to meet client needs.

Required Education and Experience:
A Masters degree in social work, psychology, or another human service-related field is required.
Must have or be able to obtain a Certified Alcohol and Drug Counselor designation.
Must have or be able to obtain certification from Oregon Health Authority as a Certified Mental Health Investigator.

Preferred Education and Experience:
Bilingual Spanish.
Experience with an electronic Medical Records system.
