AZ-Clinic Care Coordinator

Job Description:

Primary Care Clinic at Tsehootsooi Medical Center.
Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:
Requirement: Three (3) year Case Management Experience.
Shift: Weekdays Monday through Fridays, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.

Orientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.

Travelers are occasionally expected to float to other departments in which they are qualified to work.
