Job Summary WELLBE INTRODUCTION The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care - from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams' partner with the patient's primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum. GENERAL SUMMARY The Associate Director, Clinical Care Delivery role helps to support Clinical Services with field education activities of the Clinical Practice Educator team, supports clinical care activities that impact patient care, and leads CPE activities for the overall market competency testing program. S/he is responsible for following the rules and regulations of state and federal regulatory agencies and other certified agencies for their region. This position is responsible for providing direct leadership for their CPEs while providing educational and patient care leadership. This leadership role supports the company's overall business strategy and the daily implementation for all field education activities and quality patient care related functions and performance of the assigned geography and direct reports. The Associate Director, Clinical Care Delivery is expected to foster positive and effective working relations with senior leadership and management, local market management, and business ancillary departments for the purpose of achieving the company's vision, mission, and objectives. responsible for modeling the WellBe values and mission. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Company, among all departments, and all external stakeholders. This position will lead a regional team of Clinical Practice Educators. Job Description SKILLS & COMPETENCIES Develops and maintains good working knowledge and expertise of all key clinical areas to support the delivery and education for high-quality patient care, clinical practice compliance, quality functions, and patient/member experience. Supports core development of the educational content and nurse practitioner competency assessment processes. Responsible for all direct report onboarding, training, mentoring, and ongoing growth and development for their CPE team. Performs administrative activities associated with the leadership of their Clinical Practice Educator team. Provides regional educational leadership for implementing new initiatives and existing processes. Helps to train and ensures all Clinical Practice Educators direct reports are aware of and comply with company, government, and patient policies, procedures, and regulations. Continually monitors at market and regional levels for educational needs and works with clinical director and education team to identify market or business wide opportunities for educational gaps. Supports team training for APP onboarding and field education days to support new APP staff positions to meet WellBe and patient needs. Works directly with Clinical Practice Educator and Sr Director of Clinical Care Delivery to identify opportunities for clinical content development. Carries out any supervisory responsibilities in accordance with the organization's policies and applicable laws. Supports team efforts to identify any needed policies or processes to support patient care and clinical services training. Performs other duties as assigned. Other tasks needed to accomplish department objectives/goals. Job Requirements QUALIFICATIONS Educational/Experience Requirements: Valid Nurse Practitioner license with completed certification in areas of Family, Adult, or Adult/Gero. Master's Degree or higher (required) 2+ years formal educational leadership experience (required) 5+ years of NP or related nursing work experience (required) 3+ years of direct people leadership/management experience (preferred) Previous experience in home care, hospice, palliative care, or geriatrics strongly preferred, other complex patient health care experience also beneficial. Prior start up experience (preferred) CPR certification (Required). Travel requirements: Travel may be required up to 50% locally or nationally.
