Description JOB SUMMARY The Annual Wellness Visit nurse works in collaboration with the primary care provider and all health care team members. Primary responsibilities include the identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and the execution of guided processes to complete the AWV. May also help clinic staff with completing other clinical tasks as needed. Assesses healthcare needs that require additional follow-up by the primary care provider and assists patients in scheduling appropriate follow-up. Knowledgeable of community resources and assists patients in connecting with those resources. Serves as a patient advocate and assists in identifying and improving service delivery. This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations. The AWV nurse is expected to see at least 8 patients per 8-hour day. All activities will be performed in support of the strategy, vision, and values of Phoebe. GENERAL REQUIREMENTS Adheres to the hospital and departmental attendance and punctuality guidelines Performs all job responsibilities in alignment with the core values, mission and vision of the organization Performs other duties as required and completes all job functions as per departmental policies and procedures Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs) Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time. Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills. Wears protective clothing and equipment as appropriate. WORKING CONDITIONS General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels. May be exposed to high noise levels and bright lights. May be exposed to limited hazardous substances or body fluids, or infectious organisms. May be required to change from one task to another of different nature without loss of efficiency or composure. Periods of high stress and fluctuating workloads may occur. May be scheduled as needed including overtime Qualifications EDUCATION REQUIREMENTS Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing. (Required 2 year / Associate Degree (Preferred) EXPERIENCE REQUIREMENTS 2 - 3 years Clinical setting (Required) CERTIFICATIONS AND LICENSURES Required Certifications/Licensures: Licensed Practical Nurse (LPN) with current Georgia license, Basic Life Support (BLS) GENERAL SKILLS Communication Skills Interpersonal Skills Customer Relations Read / Comprehend Written Instructions Follow Verbal Instructions Basic Computer Skills.
