DESCRIPTION OF WORK As an Aging Care Manager 2 for the Assessment Unit, you will be assigned approximately 35 to 40 cases each month through the AAA intake and referral process. You will be responsible for conducting the initial field visit with consumers, their family members, and others, either in their homes or long-term care facilities, in order to determine an appropriate level of care . You must exercise strong critical thinking skills and the ability to accurately and efficiently prioritize work based upon assigned cases and the safety and needs of the client. You will assist walk-in clients with completion of forms such as PACE, rebates, transportation applications, and more, in addition to completing statistical reports, travel vouchers, and participating in conference calls, webinars, workshops, and mandatory trainings. Within this role, it is vital to maintain a current working knowledge of available community support services, as well as eligibility guidelines, and create positive working relationships with consumers, outside agencies, the general public, and other staff members. Work Schedule and Additional Information: + Full-time employment + Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. + Salary: In some cases, the starting salary may be non-negotiable. + You will receive further communication regarding this position via email. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Minimum Experience and Training Requirements: + Six months as an Aging Care Manager 1 or a County Caseworker 1; or + Successful completion of the County Social Casework Intern program;or + Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;or + An equivalent combination of experience and training.
