Duties Manage the primary care needs of an assigned population of patients utilizing evidenced-based clinical practice guidelines and patient engagement. Obtain detailed history and physical exam, performs initial patient work-up and assessment of patients' care and treatment; orders appropriate diagnostic tests; establishes a preliminary and/or final diagnosis with appropriate treatment. Assess real and potential health hazards and current health status, discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems. Provide surveillance of patient's need for preventive services and management of chronic disease leveraging information technology decision support tools and the members of the medical home port team. Conduct a variety of physical examinations at the request of the beneficiary or Defense Health Agency for pre-employment. Complete documentation in the patient's electronic medical record and hard copy, providing appropriate code level and diagnosis of patient visits using established standards. Provide emergency or crises interventions including lifesaving emergency procedures in order to stabilize a patient within the scope of practice and approved protocols. Requirements Conditions of Employment You must undergo a specific initial background checks, investigation, and periodic re-verification check of multiple agency records to obtain and maintain a suitable determination to occupy a position having regular contact with children. This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Tier 3 security investigation or access for the duration of employment. A background investigation and credit check are required. Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. This position may require the incumbent be able to obtain and maintain a determination of eligibility for a Secret security clearance or access for the duration of employment. This position has mandatory seasonal influenza vaccination requirements and is subject to annual seasonal influenza vaccinations unless otherwise exempted for medical or religious reasons (documentation of exemption must be provided upon hire). This position is subject to a pre-employment drug screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident. This position requires the incumbent to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges. This position requires that you obtain and maintain a Basic Life Support (BLS) certification. This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position. This position requires the employee to obtain Hepatitis B immunizations and Tuberculosis testing unless exempt by physician. Employee will be required to wear and use personal protective equipment as required and provided by the employer. Basic Requirement for Advanced Practice Nurse (Family Medicine): Degree: A graduate or higher level degree from an accredited professional nursing educational program in Family Medicine Nurse Practitioner is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.). Foreign Degree: Graduates of foreign nursing schools must have completed the Commission on Graduates of Foreign Nursing Schools Certification Program. The certification program consists of a credentials evaluation, the CGFNS Qualifying Exam, and evidence of passing one of the approved English Language Proficiency examinations. A copy of the CGFNS Certificate is required. Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination. AND A current, active, full, and unrestricted license or registration as a professional nurse in the specialty of Family Medicine Nurse Practitioner or related specialty from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Professional Nursing Experience: One or more full years of professional nursing experience which includes performing professional medical screening, diagnostic and treatment services; providing health maintenance and preventative services; and completing documentation in the patient's electronic medical record This definition of professional nursing experience is typical of work performed at the next lower grade/level position in the federal service (GS-11). Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone no substitution of education for experience is permitted.
