We need an Admissions/MDS Nurse to join our team at Shawneespring Skilled Nursing and Rehabilitation in Harrison, OH.
LPN: $28 to $32 Per Hour
RN: $32 to $40 Per Hour
You'll assess new and existing patients to ensure that all of their care needs are addressed and documented on the MDS.
You'll Be Responsible For:
- Overseeing the MDS process following the RAI manual
- Working with other MDS nurses to complete initial assessments for new admissions
- Properly managing patient admissions scheduling, MDS assessment & care planning
- Completing required submissions weekly or as directed by the DON
- Reviewing validation report to ensure error-free submission
- Obtaining & reviewing QM's
- Working with IDT to improve resident outcomes
- Attending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process
- Addressing the needs of residents by developing & updating comprehensive & nurse aide care plans
- Ensuring non-nursing sections of MDS & CAA's accurately reflect resident needs
- Monitoring care plans to identify resident needs
- Reviewing order recap reports & nurses' notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed
- Monitoring residents' change of status & determining need for change per RAI
- Ensuring accuracy of supportive documentation according to MDS/RAI regulations
- Working with facility consultants to implement recommended changes
- Completing AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperwork
You'll Need To Have:
- A current LPN or RN license in the state of Ohio
- A flexible work schedule & the ability to work independently
- An understanding of the MDS process & a willingness to continually learn
- Knowledge in PDPM & RUG
