The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivate and talented individual for the Acquired Brain Injury (ABI) / Moving Forward Plan (MFP) Human Services Coordinator I (HSC I) Position at the Southeast Regional Office in Middleboro. The Human Services Coordinator I position will be based out of the Regional Office in Middleboro and provides services to individuals served by the DDS Southeast Region. This position will be coordinating services for individuals eligible for ABI/MFP waivers pursuant to the Hutchinson settlement agreement. The Duties and Responsibilities of the ABI/MFP Human Services Coordinator include, but are not limited to, the following: Duties and Responsibilities: (The duties and responsibilities stated are general summary and not all inclusive.) Facilitate meetings with the interdisciplinary team to develop Individual Service Plans (ISPs). Facilitate meetings with the interdisciplinary team to develop safe transition planning for waiver participants moving into the community. Coordinate activities of a multitude of service stakeholders including: MassHealth, UMASS, Transitional Entities, and Agencies, Residential and Day service providers and DDS staff. Maintain and update written and electronic records. Serve as advocate on behalf of the individual. Monitor and documents the provision of services and modifies service plans. Protect the privacy of information and ensures that the individual's legal, civil, and human rights are protected. Provide information and other services to individuals and their families. Preferred Qualifications: Extensive experience working with individuals with acquired brain injury and related conditions. Knowledge of the types and manifestations of development, psychological and emotional disorders for individuals with acquired brain injury. Familiar with ABI and MFP Waivers. Experience transitioning consumers from a nursing / rehab facility to community-based services. Understand Medicaid and Medicare programs and regulations. Skilled at facilitating a Team process including mediation of disagreement among members. Working knowledge of Microsoft Office Suite applications and good organizational skills. Schedule Information: Monday through Friday: 9:00am - 5:00pm Days Off: Saturday and Sunday Flexibility required to meet the needs of the program. Driver's License Required.
